I've had a day to think on this, and I now believe that the digitizer is damaged.

there is a small dimple in it from it having been subjected to "a rock like" article and enough pressure applied to permanently deform the digitizer surface.

for me, that could be the top of my office desk if it is laid face down

carrying it in your pocket is a 'no no', just for that reason.
